Use the methods that you feel are the best way to portray this information. The title and reference pages do not count but need to be included.
Indicate in the paper what the difference is between primary and secondary data. Also state what is considered patient identifiable information.

Uniform Hospital Discharge Data Set (UHDDS) for hospitals
Uniform Ambulatory Care Data Set (UACDS) for ambulatory care
Minimum Data Set (MDS) for nursing facility resident assessment and care screening
Outcome and Assessment Information Set (OASIS) for home health
Step 2: Describe the similarities and differences of the four data sets following the above instructions.
