I will give you an idea about my project (just for you only before to start) I did literature review about anticoagulant safety in clinical practice. And result analysis for the data which received from addenbrooke,s hospital. clinical incident data from Cambridge University Hospitals NHS Foundation Trust (addenbrooke,s hospital ) period was obtained. This data was related to incidents where anticoagulant had been involved. An appropriately authorised member of the Trust staff obtained permission from the Trust Risk and Patient Safety Department to access data from the incident database. This data was made anonymous so that individual patients or staff members could not be identified.

Please see attached of literature review- chapter -1 and result chapter -3. Now I need to write the discussion chapter-4.

In the discussion (chapter-4) must to do critical analysis for the result chapter-3 and not only descriptive analysis ( 2700 words). Also need to focus for the main findings by doing them analysis for these findings and there incidents.( please read the incident details from the Data report , see data report attached). Please support your critical discussion for the result chapter with the evidence study from the literature review such as fanikos indicate that……..

I will give you this information, which could help you to understand some of the result chapter-3. This data was done in addenbrooke,s hospital ( Cambridge university hospital NHS Foundation trust). In this hospital, they implement computer physician order entry Also they switched enoxaparin to dalteparin in the middle of so you will find the number of enoxaparin is low. Dalteparin dose depend on patient weight so in theelectronic system of prescription still calculate the dose according to patient weight, which could increase the high risk of bleeding if patient is obese.
Warfarin need to monitor the INR and the electronic system not fix it yet to determine the previous dose of warfarin.
Dispensing has the lowest number of errors. It could be due to the apply of pharmacy error management and reporting (- pharmacy error management and reporting)
There are brand new doctors from medical school starting in august to February every year,so it could affect the total monthly or yearly of medication error in addenbrooke,s hospital.

Recommendation, suggestions for Future work and limitation ( 1500 words)
Now, after when you make critical discussion for the result chapter what are the recommendations you come with up to reduce these errors (please see attached of recommendation1 from the same hospital: addenbrooke,s hospital, recommendation2 and recommendation 3). Also add these recommendations please; to have an anticoagulant clinic in the addenbrooke,s hospital and should be conducted by clinical pharmacist who has specialist in anticoagulant medicine. Improve the electronic system such as give the anticoagulant orders different colour (red colour as example) in the screen to give high attention for all medical staff. Also use different label for the anticoagulant medicine when they dispense from pharmacy to the different wards to give more attention to the pharmacy and nurses. In addition to make quarterly audit ( every three months) for the anticoagulant errors and analyse these errors and take the suitable actions.
