Assessment and Surveillance Plan

To achieve a successful project experience and outcome, you are expected to meet the following requirements. Written communication: Written communication is free of errors that detract from the overall message. APA formatting: Resources and citations are formatted according to APA (6th Edition) style and formatting. Literature Review: Minimum of 15 resources. Length of paper: Minimum of 15 typed double-spaced pages, excluding front material and any appendices. Font and font size: Arial, 12 point. Provide an annotated bibliography of articles and other materials you plan to use for your Assessment and Surveillance Plan project. In your annotation, provide a brief (1–3 sentences) summary of the resource and a brief (1 or 2 sentences) discussion of how you intend to apply the resource’s content to your project. The bibliography must contain at least 15 sources. All sources are expected to meet academic standards; articles should be peer- reviewed and non-article material should be from reliable sources such as government or academic sites. Assessment and Surveillance Plan • Needs Assessment: o An introduction that identifies the health concern and the affected population. o A background section that contextualizes the health concern and the effect of social policy on previous interventions and programs. o A literature review that addresses the topic, including health disparities and cultural differences. o A theoretical assessment of the problem including a model. o Identification of the stakeholders, inclusive of community collaborators and private public partnerships and their corresponding roles. • Surveillance Plan: o Purpose or rationale. o Objectives. o Methods. ? Case definitions and parameters. ? Data collection. ? Data analysis. o Interpretation or dissemination. ? Communication plan. ? Marketing. ? Use. ? Evaluation.
