This assignment builds on the attached assignment (see attachments). You will be provided with:
•    resources relating to commonly used CPI tools( please use PDSA tool in this assignment )
•    A CPI template that they must type their CPI paper into.
You will be required to use a minimum of eight (12) secondary literature sources (from the last 5 years) that closely relate to their nursing issue from Assignment 2. For example:
•    systematic literature review, Cochrane review
•    practice guidelines
•    clinical protocol or guideline
•    government report
•    position paper of guideline from a reliable web-based resource (e.g. The Heart Foundation or the ARC etc).
You are permitted to use the same sources they used in the attached Assignment  if they were secondary sources.
You will be required to construct a CPI report using the supplied template (see attachments). In order to pass this assignment, students must successfully address each of the criteria outlined on the marking rubric which is available in the attachments.

Clinical practice improvement
Clinical practice improvement (CPI) encompasses the methods used to plan, implement and evaluate the impact of changes in the delivery of health services. CPI is a continuous cycle of improvement activities, rather than a one-off event.
CPI provides clinicians with formal methods and tools to improve practice with a scientific underpinning for those methods. This process is part of an ongoing cycle where identification and diagnosis of a clinical issues or problem leads to the planning and implementation of change, the evaluation of the impact of the change and further review to determine whether the interventions have been effective. In terms of clinical practice, this process is known as The Clinical Practice Improvement Model.
Before clinical improvement activities can take place it is first necessary to gather information about the quality of current clinical care. There are various methods for providing this information. These include:
•    Facilitated incident monitoring
•    Sentinel event management
•    The effective use of clinical indicators
•    Peer review meetings
•    Morbidity and Mortality meetings (M&Ms)
•    Ad hoc audits/reviews
•    Retrospective chart review
Further information about these activities can be found in the ‘Clinician’s toolkit for improving health care’ (link below).
Once adequate information has been collected about the current quality of care it is then possible to identify any problems with systems of care or with individual clinician’s practice, and begin to plan changes that will result in an improvement in the delivery and quality of health care.
It’s important that potential changes to delivery of health services are carefully planned and considered. Several tools can help you successfully manage change and implement improvement strategies, including:
•    PDSA cycle (the most commonly used tool) Please use this tool in this assignment
•    PEPPA framework.
It may seem unusual to test a change before you’ve implemented it, but this approach helps you save time, enhance patient safety, reduce disruption for patients and colleagues, and gain valuable learning.

Case Scenario
You have been dispatched at 11.00 to an 18 year old female with shortness of breath. She has been assisted by two of her University lecturers and two classmates to their office and an ambulance called. She states that she was feeling very nauseous and unwell this morning and has had little sleep over the last 3 weeks. She has been to see her local doctor three (3) times in the last four (4) weeks. Her doctor has conducted blood tests, a chest x-ray and an abdominal ultra-sound. She is awaiting the results.
Over the past 7 weeks she has been increasingly short of breath, it seems to be worse when she goes to University and when she tries to go to do her afternoon shift at the pub on Friday evenings. She recently had to take three (3) weeks off work because she has started to shake and feels like vomiting every time she tries to leave her home.
She lives with her best friend in a shared rental accommodation. Her best friend has noticed that she has progressively lost her energy, finds it more difficult to look after herself, and has lost interest in the band she played music with on Sundays. She states ‘she feels so tired and exhausted that she feels she is letting herself and others down’.
