Case:  You and a good friend have recently joined an intermural soccer team here at Missouri State University.  Both standout soccer players in high school, you have

each relished the chance to compete on the field again, and take the games very seriously.  Having always hated wearing shin guards in high school, your friend has

enjoyed playing without them in recent days, explaining “I can run much faster without them, and I don’t have coach yelling at me about it anymore.”  While playing for

a chance at the playoffs, your friend takes a hard kick in the front of the leg from an opponent.  Your friend bounces back up initially, though in pain and tries to

play on.  Discouraged, you friend makes it to the sidelines and a play is stopped for a moment.  Your fiend describes the injury as a “really bad cramp” that worsens

when they pull up (dorsiflex) their toes.  The leg appears swollen, tight, and pale in color.  Still your friend tries to play through the pain.  In the next few

minutes, it is obvious your friend is in severe pain and has to return to the sidelines without much movement of the foot.  They are intent on watching the rest of the

game though you begin to become nervous about their health.  Given your knowledge of anatomy, you ask you friend a few questions to ease your mind.  You ask them to

dorsiflex the foot, which they cannot do.  You find that they cannot feel anything between the first and second toes, and the leg still appears swollen anteriorly.

With only one substitute player for your team available, you are needed back on the field and your friend assures you it is just a really bad cramp.  Other players are

calling for you, and you must make a decision quickly.
Goal:Given the above information, and your knowledge of anatomy, explain whether you should pull yourself from the game (effectively forfeit) and seek emergency

medical help for your friend, or wait to the end of the game and help with the “cramp”.  Next explain which muscles are involved in the injury if your friend is unable

to dorsiflex.  Explain the loss of sensation between the toes, and trace the pathway of this (contributions to the) nerve throughout the lower extremity.  If an artery

is involved in the injury explain most likely injured artery and one way you could evaluate its status.  Lastly, state one possible condition that could be attributed

to the above symptoms seen in your friend.
