This case study is to be applied specifically to your assigned community as listed below.

Assignment details:
Please address all three parts of this assignment in your answer. Use the electronic assignment template provided, and name each part of your essay using the headings below.
Ensure you address all points, referring to sources of information whether provided from within the unit website or sourced through your own research. Reference your sources correctly using the APA 6th referencing guidelines.
Part 1 –Populations and health
(Approximately 1,000 words, 35% of grade)
Research and describe the health needs of your assigned community. Your description should include:
• A range of health and related services available in that extended community (10%)
• At least two cultural groups or special populations which exist in your assigned community and how people within these groups would differ in their need for health services (15%)
• Social determinants of health and how these impact the health needs of your assigned community’s population groups (10%)
Part 2 – Workforce and service models
(Approximately 850 words, 35% of Grade)
Describe existing models of service delivery and suggest alternative models that could be used to address the particular health needs of your assigned community, taking a biomedical approach. Give examples. (15%)
Describe existing models of service delivery and suggest alternative models that could be used to address the particular health needs of your assigned community, taking a primary care approach. Give examples. (10%)
Research and describe the medical workforce in your assigned community and discuss the impact of one general practitioner (GP) doctor leaving their practice without any replacement. Compare this with the impact of one GP leaving their practice in a town classified as ASGC-RA 2.
Also describe recruitment and retention strategies that might sustain the medical workforce in your assigned community. (10%)
Part 3 – Rural and Remote Practice
(Approximately 650 words, 30% of Grade)
Imagine yourself in the role of a health professional working in your assigned community. Please identify what role you have chosen for this case study.
Also, select one of these case scenarios to address this question:
• A baby with recurrent infections
• An adult with newly diagnosed type II diabetes
• A teenager who has just attempted suicide
To answer this question, describe how “real life” practice might look for this particular scenario of health professional and patient, being sure to cover the following topics (20%):
