To demonstrate your facility with the case theory method, prepare a draft paper (of no more than seven pages in length) using the Nancy Troy Case Identify your assumptions (the primary clinical concepts you see) about Nancy Troy based on the assessment material provided in the case. Provide a rationale for why you believe each of the identified concepts is important, using practice-based and evidence-guided literature to support your rationale. From your clinical concepts, identify the hypotheses you believe are important. You should use a minimum of eight cited resources for all of your concepts of which only two resources can be from the required readings in the modules. Think of the paper as a case analysis where you will show how you 1) identify primary clinical concepts (or assumptions) about the case, 2) support those assumption with relevant resources, and 3) form specific hypotheses and interventions from these concepts about the case.

The Bisman article from week 3 (Social Work Assessment: Case theory construction) does a great job of explaining this process in detail and gives an example of how this works with the case of Rosie. For example, a clinical concept about the case of Rosie is:

“Rosie’s recent HIV diagnosis is exacerbating her long term social isolation and possible clinical depression.”
Bisman backs this up with resources to support this concept:
“Goldstein (1995) and Jue (1994), indicate that stigma from AIDS often socially isolates these patients, while Mancoske (1996) points to their greater risk of suicide. Individuals need an active energy exchange with others as Greene (1 991) explains in her discussion of systems theory, and for a long time Rosie has had no person with whom she can talk openly. Her long history of severe mood swings suggests bipolar disorder. Evidence supports a biological basis for treatment of depression (Sperry, 1995). “
This lead to Bisman’s hypothesis that:

“Rosie’s symptoms are explained by her biology, social isolation, and disease…. Intervention should include both increasing social supports and a medical consult to consider pharmacology for the possible bipolar disorder. Focus on just AIDS, or only the social isolation, or solely the clinical depression ignores important variables. Janet is aware that any of these narrow interventions is potentially harmful to Rosie.”
