4. Any acronyms used to refer to the instrument (if applicable)
5. The instruments author(s)
6. Key reference(s) for understanding the instrument
7. The purpose of the instrument(s) (i.e., what it will measure)
8. The rationale for your selection and development of the instrument(s)
9. The number of items to be examined
10. Any relevant subsets or subscales and their definitions
11. The response format (e.g., Likert, multiple choice, yes/no, open ended)
12. How the instrument is scored
13. The reliability of the instrument
