Please answer any 3 of the 5 questions below. Answers for each question can be drawn from in class lectures, take home presentations, book readings and out of class research. Answers are essay in format, so I’m looking for the why’s and the how’s vs. simple definitions. Demonstrate what the real world examples would be, how can this be applied in your personal work / student environment. The more detail the better. Remember, you’re trying to demonstrate to me that you understand these concepts and this will translate into more successful results once you begin to implement in your workplace.

Examinations are to be “take-home” and will be handed-out one week prior to the due date. Examinations are to be completed on an individual basis. Plagiarism will not be tolerated, and is subject to a failing grade in the course. Proper credit must be given to all works cited. Students found cheating on examinations will be dealt with as outlined by university policy.

1. What is process for performing a Single Minute Exchange of Dies (SMED)? What are the two types of time, what is the goal? What are some of the pitfalls of setup? What should the ideal setup time take? What are some of the other Lean tools available to implement a successful SMED Kaizen / why?

2. In PDCA Problem Solving, there were 4 Phases, what were they and why is it so important to follow them. What is different from PDCA Problem Solving versus other Problem Solving techniques, think about the P D C A portions and what separates PDCA Problem Solving versus others?

3. We discussed the relationship between Lean and Six Sigma tools, why is so important to achieve both? There were three different curves, what does each represent and which one would an ideal process have? What are the Costs of Poor Quality (COPQ), what do they mean? What are the six M’s that are on a CE diagram, what does each mean?

4. In reference to the Statapult Exercise, why was following the 6M process so important? What does that logic force in our thinking and behavior? What could we have done different to improve the accuracy? What could we have done different if we wanted to hit a specific distance, regardless of the limitations provided?

5. What are the different types of behaviors exhibited when we Facilitate, why is important to recognize these? What could we do to counter them? Give me an example of each in your personal examples and how did it interfere with your end result?
