Family Assessment Assignment

This assignment is a forum for students to describe, challenge and learn from each other. It is also an opportunity to utilize knowledge gained in class and develop health promotion, disease prevention and family assessment skills. Completion of the assignment is a significant step in addressing the health needs of the family.

INSTRUCTIONS FOR ASSESSMENT & INTERVENTION:
For this project, each student must complete the assignment during two home visits using the following instructions. A follow-up phone call or additional visit may be indicated to evaluate the effectiveness of the intervention. Students must complete & submit a written report of their family assessment & intervention, and present their findings in clinical.

ASSESSMENT
1. Complete a home safety/home environmental assessment using the tool provided

2. Assess and describe the family’s level of Emergency Preparedness

3. Assessment – Provide a brief summary about the family including the following information:
a) Type of family
b) Describe the family functions
c) Describe the family structure – use a family assessment tool (genogram, ecomap, etc.)
d) Use all 3 three Theories for Working with Families in the Community to describe the family (Family systems theory, Family development and life cycle theory, Bioecological systems theory)
e) Is the family healthy or dysfunctional?

4. Health Risk Appraisal – Conduct a family health risk appraisal assessing for the presence of specific factors in each of the categories that have been identified as being associated with an increased likelihood of an illness (Biological/age-related health risk, Environmental risk, Behavioral risk)

5. Identify at least 5 resources/strengths the family possesses.

6. Identify and list at least top 5 family needs. Prioritize the identified needs.
INTERVENTION
7. Design a detailed culturally appropriate intervention plan for 1 or more of the needs identified and list specific public/community health nursing roles you would employ in addressing the family need (from intervention wheel – home visits, screening, case management, etc.). Include the level of prevention you used (primary, secondary or tertiary).

8. Review literature and select one article from the literature in support of the identified intervention. Use the article to support your design and attach the article.

9. Implement the intervention with the family, providing appropriate resources as indicated.

10. Evaluation- Develop an evaluation plan for the intervention and include recommendations for the future. Follow-up with the family to conduct an initial evaluation of the intervention.
GUIDELINES FOR ASSIGNMENT
1. Answer all the questions
2. Resources: Use the textbook (Chapters 27 & 28).
3. Home Visit Process:
• Initial phase
• Pre-visit phase
• In-home phase
• Termination phase
• Post-visit phase
• Five skills: Observing, listening, questioning, probing, and prompting
4. Home Safety:
• Home safety includes evaluating the client’s home care environment for fire risk, environmental hazards, and other elements that present a risk of accident or injury to the client.
• Involves home modification recommendation (lighting, side rails, etc.)
• Includes teaching clients self-care, preventive measures for home care, etc.
5. Making an Appointment:
• Remember that the assessment is reciprocal and the family will be making judgments about you when you call to make the appointment.
• Introduce yourself and the purpose for the contact.
• Do not apologize for contacting the family. Be clear, direct, and specific about the need for an appointment.
• Arrange a time that is convenient for all parties and allows the most family members to be present.
• If appropriate, ask whether an interpreter will be needed during the meeting.
• Confirm place, time, date, and directions.
6. Interviewing the Family:
• Start with informal conversation.
• Involve each family member in the conversation.
• Shift to formal interview by asking the family to share their story about the current situation.
• Purpose of the interview is to gather information and help the family focus on their problem and determine solutions.
7. Interviewing the Family – Therapeutic Questions:
• What is the greatest challenge facing your family now?
• On which family member do you think the illness has the most impact?
• Who is suffering the most?
• What has been most and least helpful to you in similar situations?
• If there is one question you could have answered now, what would that be?
• How can I best help you and your family?
• What are your needs/wishes for assistance now?

8. Designing Family Interventions
• We need the following type of help.
• We need the following information.
• We need the following supplies.
• We need to involve or tell the following people.
• To make our family action plan happen, we need to… (List five things in the order they need to have happen.)
9. Evaluation of the Plan
Determine whether:
• The plan is working
• The plan is working fast enough to address the problem
• The plan is addressing only part of the problem
• The plan needs to be revised based on changes
If plan is not working, nurse and family work together to determine the barriers interfering with the plan
10. Submit project as instructed through blackboard and present the process in class.
