Health History and Physical Examination
Paper #2: Comprehensive History and Physical Paper (30 points) Student will find an individual with at least one (1) chronic disease in his/her family/community. This individual could not be a classmate or a patient in the hospital. The problem list needs to include at least five (5) problems. At least two (2) Nursing Diagnoses should identified: one medical (chronic disease) and the other psychosocial. The plan of care for the two (2) diagnoses should be written. A sample paper is posted on pg. 809-813 in textbook. Papers must be submitted as a hard copy and via Moodle on assigned date.
