In the first weeks of this course, you have identified a health promotion issue and identified a potential intervention. You have also completed a comprehensive review of the literature and now will integrate that information to create an intervention designed to address the health promotion issue. As you create the intervention, try to keep your focus on an intervention that you will be able to evaluate, which is the next and final step in the project. Be sure to include all feedback from your instructor in each step of the project.

The Topic of my health promotion proposal is ( Prevention of hypertension and stroke among the immigrants from West Africa, who come from a healthcare underserved area and live in Dallas Texas . Main complaint is that they are not able to count calories or control portion due to food differences. These immigrants are in the age bracket of 40-50 males and females, that area at risk for hypertension. There have been increased death recently due to seeking medical attention right before death. My intervention is to go use the local church as a medium to reach out to them, providing information about resources available, flyers, local social media, holding health fair periodically, to evaluate progress, providing record cards to track monitoring and screening of Vital signs, cholesterol and providing general and focused teaching as well as danger signs. However my main intervention is a meal after the church service using a DASH recipe

Your project has good bones but needs some definition.
Your cohort is age 40-50 African-American males and females from the Dallas area with an ICD code of pre-hypertension. You want to say in your paper where you have identified the population i.e. obtained ICD code xxx and Africa-American and no prior cardiovascular events from a large internal medicine practice(s) in Dallas. Once you have identified this population, what will you do with them?
Please listen to my audio. Per Nikitas, our text, people cannot make multiple lifestyle changes at the same time. Your paper mentions DASH, weight loss etc. Please see line item 2 in the rubric … a specific, narrow and focused intervention to address the goal. Perhaps you want to focus on DASH cooking night, or a walking club, or a stress management program. The ideas are endless, but please select and “run with” a specific, focused and narrow intervention. I hope these ideas help
I agree that the community Organization Model is a good one, although you also mention Leninger’s cultural sensitivity. Perhaps you could clarify in your paper to say that your health promotion project is underpinned by the Community Model, with cultural competence and therapeutic relationship aspects guided by the work of Leninger.

The highlights are that you mention both the Community Model and Madeleine Leninger’s Model, and I provided you with some ways to clarify using both in your paper (see your email). I would encourage you to develop your intervention. This paper mentions pamphlets. These are one of the least effective methods of communication, and the APRN must be in the midst of the intervention. I suggested cooking a DASH meal for the church population. You also want to consider how to narrow your population. Not all people 40-50 in the church will have risk factors for HTN. Perhaps you want to use one of your score cards to select your cohort for your intervention. Brochures are one of the least effective ways to reach people (on my audio), rather you want to engage the church people perhaps with a meal after the church service using a DASH recipe
I agree that the report cards with biometric data can be used, but should be used pre and post test, and perhaps consider the pre-test to screen out those who may not have HTN in the church.
