Component 1: Case study (50% of module mark)

Guidelines
 There are 2 sections to this case study (see below for details).
 Section 1 should be 1000 words maximum (including the 174 words in the template).
 Section 2 should be 1000 words maximum (including figure headings).
 An excessive word count will result in a penalty.
• 5% will be deducted for each 200 words over the 2000 word limit.
 A word count must be included at the end of the document.
Section 1: (50% of mark)
On WOLF, in the ‘Coursework assignment information’ folder, you will also find a blank patient assessment form. You should complete this form to ‘create’ your own patient with their relevant medical records based upon your selected disease.

You should complete all sections of the patient assessment form. An example of a completed patient assessment form can be found on WOLF (however this does not contain reference citations, yours should).

The patient should represent a stereotypical example of a person who you would expect to present with the disease. As such you need to research the epidemiology of the disease; are they more likely to be male or female, would there be a family history of the disease, would they be older or younger, what would be the risk factors (e.g. unhealthy lifestyle and diet) etc.

Are they also likely to have other conditions associated with the presenting complaint? If they do have another disease they may already be on medication, you would need to include this.

What are the clinical manifestations of the disease; what symptoms would you expect the patient to present with? What physical examinations could be done simply at a GP’s surgery, for example BP monitoring, temperature reading, taking a urine or blood sample.
What further investigations would need to be completed to diagnose that disease and rule out a differential diagnosis?
What would be the action plan, for example, treatment or further investigations.
Reference citations should be included throughout. References used to retrieve the information for this section should be included at the end of this section. Citations and references do not count towards the total word count.

Section 2: (50% of mark)

Discuss the pathophysiology of the disease presented in your patient assessment form.

• Explain the pathological processes that occur within the body which result in the signs and symptoms of the presenting condition.

For this section reference citations should be included and a reference list at the end of the section.
Figure headings are included in the word count.
Citations and references are excluded from the total word count.
Referencing

Accurate referencing is commensurate with good academic practice and enhances the presentation of your work: it shows that your writing is based on knowledge and informed by appropriate academic reading.
Referencing ensures that anyone reading your work can trace the sources you have used in the development of your work, and give you credit for your research efforts and quality.
If you do not acknowledge another writer’s work or ideas, you could be accused of plagiarism.

When, in your work, you use an idea from a book, journal article, etc., you must acknowledge this in your text. This is referred to as ‘citing’. When you insert a citation using the Harvard system you must always state the author/editor and the year of publication. If the work has two authors/editors you must include both names.
You do not need to include the title, place of publication, etc.; the full details of the work are written in your reference list, which you should include at the end of your assignment.
You will be marked on two aspects
 Style
 Correct use of the Harvard referencing system
