In project part 3 The Flow chart does not show a complete process – the scan post discharge file step does not seem to have any connection arrow where its coming from. Include the steps to from collecting/secure medical records post discharge, confirm receipt of all appropriate records, actions to resolve missing records, steps to complete the scanning process.
The Gantt chart should be done in excel to reflect the actual timeline of steps – examples below
