Explain which principles are used to find accident/incident causes (FTA, Events and Causal Factors) and which are used when presenting data in graphical format to find trends (normal and poisson distribution) There are different methods used to determine the factors that lead to an event such as the FTA analysis and Events and Causal factors. A fault tree analysis (FTA) is a systematic method that examines a system using a top down approach and provides graphical diagrams for easy understanding. The key elements are the ‘gates’ that represent the outcome while ‘events’ represent the input to the gates. The events are the meaning while the gates are the conditions. The FTA diagram uses a graphical method with pathways that connect within the system. The pathways are used to connect contributory factors which have the potential to cause undesirable loss or failure. The contributing factors are connected with standard logic symbols such as AND, OR among others. The FTA principles are used to find the causational factors leading to an event and find establish a pattern of how factors can lead to the same outcome. The FTA analysis identifies failures using a deduction method helping the team focus on the cause of each event. By highlighting the important elements the organization is able to prevent a single component that could be leading to different incidents. The FTA analysis focuses at one fault at a time helping the team to stay on track. The events and causal factor analysis (EFCA) serves three main purposes in an investigation; verify causal chains, provide a structure to integrate the findings of an investigation and aid communication through the process. Written and verbal communication has to be clear and concise preventing chances of it being interpreted differently by different people. It is important to write with the audience in mind hence tailor the information to the level of their understanding. Responding to the needs of other requires one to empathize with them, learn their behaviors and culture and customize your response to their needs. Diagrams such as pie charts histograms and graphs are used to indicate relationship between contributing factors and an incident and patterns of occurrence. Statistical and epidemiological analyses are used to describe causational factors that lead to ill health in workplace environment.
