These are the social determinants of health impacting the north end community include the following as identified by our group: This paper is to provide a very critical perspective to the collaborative intervention developed by our group.
Social exclusion – feeling left out of the rest of society lack of accept to participation in meaningful activities
Race – majority of the residents of the north end community are people of color and Aboriginals
Gender – many of the residents are women who are stressed, overworked, overburdened and poor
Unemployment – there is very high unemployment rate in the north end community and the job insecurity is very high as well
Lack of food security – inadequate access to healthy meals on a regular basis (lots of north end community members make constant use of food banks and food vouchers)
Homeless – Lack of affordable and safe habitable houses
Disability – includes physical and mental disability
Education – insufficient knowledge about chronic disease management
Health care services – Access to quality and affordable health care services

What are the strengths and weaknesses in addressing the social determinants of health in the north end community as shown in the video?
What did the collaborative intervention take into account/consideration? We took into consideration that the vast majority of the residents of the north end community are in the low income group and they are mostly overburdened, over stressed and overworked and underpaid.
What other social determinants of health are there in the north end community?
The analysis must include/highlight the manner in which a critical perspective ought to inform clinical or community practice related to the collaborative intervention?
In expanding the group intervention, examine where or not the collaborative intervention utilized a clear theory or theoretical approach to the intervention. If so – is the theory clearly stated and why was the theory chosen? If not – what would you identify as the underlying theory or theories that is influencing the collaborative intervention chosen? Provide a minimum of 8 references related to your intervention and they must be different from any others cited in the groups wiki or work.
The group of social workers will provide education on community resources, referrals, service navigation and brief counseling. Social workers can also aid community members in accessing non-profits organizations and government programs/services as well as help address the community members concerns of social isolation and provide collaboration on increasing job security and prevention of job loss. They can also advocate for access to shelter and facilitate access to appropriate resources like child protection, social assistance, employment and locating and securing affordable housing. Is this a theoretical approach to the intervention, if so, is the theory clearly stated and why was this chosen? The group of nursing students worked on providing vaccinations, delivery and administration of medication, risk management, provision of medical advice and information, aftercare plans and follow up.
The group (nursing and social workers) also discussed funding options for suggested programs as well as boosts for programs already in place. Additional services/programs suggested are as follows:
Social determinants of health (SODH) screening
Health Education
Support for services/program navigation
Case Co-ordination

The north end community health Centre already has a multidisciplinary health care team on staff. Our group recommended the hiring of an additional nursing staff to support the use of SODH screening and one more social worker to support the office team already in place for this pilot project.

What are the pros and cons of working in collaboration with others, what are the challenges?
We carried out a survey and came up with the following questions:
What services are most least useful?
What resources are still needed?
Whatare our biggest concerns on a daily basis?
Do you have additional feedback?
Can this service model be replicated in other communities similar to the north end community?
Client follow up will be a common way to maintain relationship
