Gaining public health funding is often a challenge. Public health nurses may be tasked with helping write grants because they have an in-depth understanding of the needs of populations and clinical knowledge to articulate how those needs can be met.

- Person(s)/role/agency to complete the evaluation
- Frequency of evaluations
- Description of how the evaluation results will be used
- Budget
- Budget table, including items, amount, and justification
- Long-term funding plans following the end of the grant cycle
